ElderLink has been providing care management to elderly Northern Virginians since 1991.

  • Caregiving Solutions for the Elderly and Their Families

    • Created by a unique partnership among the Fairfax Area Agency on Aging, Inova Health System, and the Alzheimer's Association, ElderLink provides affordable care management services.  As a not-for-profit organization, ElderLink utilizes a state grant to provide services on a sliding scale fee system to those who are eligible.  Others can receive care management at an affordable hourly rate.

    • An exceptional array of resources, programs, and services, combined with a well-trained staff of skilled geriatrics specialists make us uniquely qualified to address the specific needs of caregivers and elderly clients alike.

    • Most importantly, ElderLink coordinates and manages all elements of patient care (medical, social, legal, financial), minimizing the stress and confusion families often experience when dealing with multiple services providers.



  • Services Provided by ElderLink - As a leading elderly care management service in Northern Virginia, ElderLink:

    • Serves all members of the older population and specializes in Alzheimer's disease and related memory loss disorders.

    • Provides a thorough assessment of the client in the home to determine what type of care is needed; develops a plan for meeting the needs of the client and family; assists in obtaining the needed services; and coordinates the services and care.

    • Guides families through the maze of government, legal, and insurance forms and procedures.

    • Works closely with family physicians and other health care providers to deliver the most effective plan of care.

    • Assists in locating an appropriate long-term care facility when the older person's needs cannot be met at home.

    • Recommends support groups for counseling families to understand and overcome the emotional issues confronting them.

    • Handles out-of-state transfers.

    • Consults regularly with caregivers and responsible parties on the client's status and makes recommendations when needed.
