Created by a unique partnership among the
Fairfax Area Agency on Aging, Inova Health System, and the
Alzheimer's Association, ElderLink provides affordable care
management services. As a not-for-profit organization,
ElderLink utilizes a state grant to provide services on a sliding
scale fee system to those who are eligible. Others can
receive care management at an affordable hourly rate.

As a leading elderly care management service in
Northern Virginia, ElderLink:

Serves all members of the older population
and specializes in Alzheimer's disease and related memory loss
disorders.

Provides a thorough assessment of the
client in the home to determine what type of care is needed;
develops a plan for meeting the needs of the client and family;
assists in obtaining the needed services; and coordinates the
services and care.

Guides families through the maze of
government, legal, and insurance forms and
procedures.

Works closely with family physicians and
other health care providers to deliver the most effective plan of
care.

Assists in locating an appropriate
long-term care facility when the older person's needs cannot be
met at home.

Recommends support groups for counseling
families to understand and overcome the emotional issues
confronting them.

Handles out-of-state
transfers.

Consults regularly with caregivers and
responsible parties on the client's status and makes
recommendations when needed.
