Adult Care Network is an easy-to-use low- or no-cost plan of services offered in the community for which you, or an elderly person you know, may qualify. The goal of the Adult Care Network is to keep the person in the community as long as possible with the necessary support.

A complete evaluation of your needs by a
trained expert, such as a social worker, a nurse, or a counselor,
depending on your situation.

Arrangement of services by a case manager to
help the family determine what services you may need, such as:
- help with daily living activities, such as eating, dressing, bathing, transferring, toileting, and bladder or bowel control;medical care;adult day programs and activities;short-term relief for the family member who takes care of you;counseling;transportation;
- housecleaning and home repair.
Who is Eligible and How Do I Obtain Services?

A member of the Adult Care Network team will talk with you about the help you may need. If you need help with daily living activities (eating, bathing, dressing, etc.), then you and your case manager will decide which services you need.
The first evaluation to determine if you need Adult Care Network services is free. If you qualify and would like to receive services, there may be a fee based on your ability to pay.
We encourage people of non-English speaking cultures to apply. The Adult Care Network has case managers from different cultures who speak languages other than English.
