TextExpired or Lost DTV $40 Coupons Can Be Replaced
Persons whose digital television transition (DTV) coupons have expired - or which never arrived or have been lost - may request replacement coupons. Replacement requests must be made by July 31. Computers will check while the person is on-line to determine if the household at that specific address has used any of its coupons (0, 1, or 2) and whether the coupons have actually expired. If applicable, replacement DTV coupon(s) will be approved; they will arrive in 3-4 weeks and will expire after 90 days or September 30, whichever comes first.
To apply for replacement coupons, the requestor must provide name and address on-line at www.DTV2009.gov or by phone at 1-888-388-2009. Original reference or coupon numbers are not necessary.
