The Fairfax County Consolidated Comunity Funding Pool FY 2015 - FY 2016 Request for Proposal (RFP) officially closes on December 3, 2013, at 2:00 p.m.
The links provided below contain the forms for the official RFP# 2000000828 Application. You may download these forms to your personal computer and add the applicable content using the software applications for Microsoft WORD and EXCEL. These completed forms may then be printed and compiled into the final application.
Any modification to the information provided in the forms is grounds for disqualification of the application.
To download the the required forms, please click on each link and download each file to your personal computer. This will enable you to enter responses directly on to the form, save it to your computer, and print the completed documents.
Microsoft Word form instructions: Follow this process for the forms provided below. Once the forms are completed, please follow the instructions in the full RFP application to assemble the required package.
1. Click on the link.
2. A window will appear. It will give you the option to open or save the document to your computer.
3. Click "Open."
4. Once the document is open, click on "File." Select "Save As" and save the document on your hard drive.
5. Fill out the form.
6. Save your changes.
7. Print the form.
Form 1 - Proposal Cover Sheet - This form must be completed by all applicants. Attach the form to the proposal, read and sign. Proposals submitted without a signed cover sheet will not be accepted.
Form 2 Proposal Narrative - This form must be completed by all applicants, with the exception of those propsing Affordable Housing Projects. Affordable Housing Projects must use the following form titled "Form 2 Proposal Narrative-Affordable Housing Projects." Follow instructions as directed.
Form 2 Proposal Narrative- Affordable Housing Projects - This form must be completed by only those applicants proposing Affordable Housing Projects. Follow instructions as directed.
Form 3 Project Outcome Worksheet - This form must be completed by all applicants. Follow instructions as directed. Please complete a Project Outcome Worksheet for each of the outcomes being proposed. Include projections for both fiscal years.
Forms 4, 4A, 4B, & 5 - Program Budget / Program Personnel Budget / Program Budget Justification/Estimated Program Revenue - These forms must be completed by all applicants. These are EXCEL documents and will automatically calculate all total lines. The document is protected and only those cells in "Yellow" can be modified by the applicant. Complete all relevant line items.
In Form 4A, list all personnel by position, the proposed salary for each position and the personnel budget justification. The total will automatically transfer to the Program Budget Form 4.
In Form 4B, provide Budget Justification. Budget line items will automatically transfer to this form.
In Form 5, provide resource justification.
Reference the Proposal Checklist to ensure that all required proposal components are included in the application.
All applicants must sign and include the “Certification of Financial Solvency for Nonprofits’ as part of their proposal submission.
RENTAL HOUSING CAPITAL PROJECTS ONLY

Capital Projects ONLY
These forms must be completed by Rental Housing Capital Project applicants only. Follow instructions as directed.
Form 11 - Detailed Estimate of Development Costs This is an Excel document and will automatically calculate subtotal and total lines. The document is protected and only those cells in "Yellow" can be modified by the applicant. Complete all relevant line items.
Form 12 - Detailed Estimaet of Annual Operating Expenses This is an Excel document and will automatically calculate subtotal and total lines. The document is protected and only those cells in "Yellow" can be modified by the applicant. Complete all relevant line items.
