Request for a Trial in Accident Cases

If you have been charged with a violation involving an accident and you wish to plead "not guilty" and have your case heard before a judge, you must notify the court within seven days of receiving the summons and your case will be continued to a new court date so that the officer may subpoena witnesses. Print this page and complete the information provided below to notify the court. You must include on this printout the following information: your court date given on the summons, the summons number, your name, your driver's license number, your daytime phone number and the officer's name and badge number. If this completed page is submitted timely, you do not need to appear on your court date as the case will be continued so that the officer may subpoena any witnesses. You will be mailed a continuance notice to the address on your summons unless the Court is notified of any changes.

If you wish to plead "guilty", you may appear on the trial date listed on your summons. If you fail to notify the court and do not prepay or appear on your scheduled date, you may be tried in your absence.
