MR Case Management Services
Case Management is an intervention which ensures that
service systems and community supports are responsive
to the specific, multiple and changing needs of individuals
and families. Case Management Services ensures that individuals
are properly connected to, and involved in, the appropriate
services and supports in order to maximize opportunities
for successful community living.
Case Managers assist in gaining access to needed homes
and jobs, social service benefits and entitlement programs,
therapeutic supports, social and educational resources,
and other supports essential to meeting basic needs. Individuals
who are recipients of Medicaid benefits receive a full
cadre of case management support, such as interdisciplinary
team planning, coordination of services, intake and assessments,
advocacy and resource planning. Those who do not have
Medicaid may also receive the same or similar service
coordination based on need. In addition, those who
are in need of emergency assistance, pre-admission and
pre-discharge planning from state training centers or
hospitals, or who are recipients of Medicaid services
will be prioritized for case management assistance.
MR Residential Services
Residential Services provides housing and residential
support services for individuals with mental retardation
in Fairfax County and the Cities of Fairfax and Falls
Church. The services provide a variety of residential
supports designed around individual needs and desires,
with an emphasis on providing opportunities for full inclusion
in community life. There is a lengthy waiting list for
residential services, which include:
Residential Group Homes provide small-group
living arrangements for individuals in neighborhood communities.
Staff support services are available on a 24-hour basis
and concentrate on developing supportive relationships,
independent living skills, and a network of friends and
opportunities in the community. Goals for all individuals
in group residences include increasing personal independence,
promoting maximum inclusion in the community, establishing
positive and natural friendships, and attaining supported
living opportunities which reflect individual choice and
desires.
Residential Support Program provides services
to individuals living in apartments, townhomes, and other
residences which may be agency-leased or individually
leased/owned. Support services range from daily to drop-in
visits and are based on individual needs. Consumer preference,
choice, and independence is encouraged. Individual and
group counseling, training and assistance in community
living and personal skills, and linkage with other support
networks in the community is also available through staff
support services.
MR Employment Services
Cooperative Employment Program provides supported
competitive employment services to eligible consumers
with developmental disabilities. The Cooperative Employment
Program is jointly funded by the Virginia Department of
Rehabilitative Services and the Fairfax-Falls Church Community
Services Board. Using an individualized approach, consumer
skills are assessed, jobs analyzed, and on-the-job training
for consumers and disability awareness training for employers
are provided. Extensive follow-up services are also provided
to ensure the success of the job placement. In addition
to the job training component, the Cooperative Employment
Program offers mobility training to enhance consumer ability
to use public transportation.
Fairfax County also procures services with private agencies.
Fairfax County contract management staff monitor all contractually-provided
services, as well as provide guidance, direction, and
technical support to those contractual service providers.
These private agencies provide an array of services
including day support, sheltered employment, community-based
individual and group employment.
MR Training and Community Support
Services
Community Outreach, Training, Quality Assurance, and
Staff Development Services provide training and technical
expertise increasing skills, knowledge, and abilities
in the areas of mental retardation programming.
Family, consumer, and community education topics are presented
to educate all concerned on mental retardation services
and how to effectively access these services.
MR Family and Community Support Services
Respite: The Respite Subsidy Program offers assistance and information regarding provider resources for families in need of temporary relief and/or assistance
All respite services are provided under a contract with
Hartwood Foundation, Inc.
Family Support Program eases caregiving demands
and assists in providing needed community supports or
services for children and adults with mental retardation. Eligible
consumers and families may apply for limited financial
assistance for needed services or supplies. Families whose Federal Taxable Income is $82,000 or less are given priority. Information,
referral to community resources, or speakers to address
community groups are also available.
