ElderLink's professional care managers provide:

How we can help.
Our first step is meeting with you and your family member to discuss your concerns. After completing an assessment, we develop a care plan that balances lifestyle with safety needs to maintain independence.
When additional services are needed, our care managers help you find appropriate and affordable services in Northern Virginia. We can also help you find a nursing home or assisted living facility if desired.
We monitor care and coordinate with other service providers, adjusting the care plan as needed.
ElderLink charges an affordable flat fee for Fairfax County residents. For all others, a reasonable hourly fee is charged.
ElderLink is a nonprofit partnership program of the Fairfax County Department of Family Services, Division of Adult and Aging Services, Fairfax Area Agency on Aging, Inova Health System, and the Alzheimer’s Association, National Capital Area Chapter.
