Fairfax County Inventory of Historic Sites - Nomination Forms

Nomination forms are available in Portable Document Format (PDF). Adobe Reader version 6.0 or higher is required to view or print these forms. The Nomination forms have fill-in text boxes for your convenience. Changes made to these forms can be saved with the Adobe Reader. Individual Property Nomination Form (PDF Fill-in Form) District Nomination Form (PDF Fill-in Form) Applicants must submit either an Individual Property Nomination Form or a District Nomination Form, with accompanying documentation, to: Historian Fairfax County Department of Planning and Zoning 12055 Government Center Parkway, Suite 730 Fairfax, VA 22035 The Historian reviews the application and submits it to the History Commission for a vote at an upcoming meeting. A file for each resource on the Inventory is kept at the Fairfax County Department of Planning and Zoning. Instructions for the Individual Property Nomination Forms are now available here.
