Fairfax County Inventory of Historic Sites - Nomination Forms


Nomination forms are available in Portable Document Format (PDF). Adobe Reader version 6.0 or higher is required to view or print these forms. The Nomination forms have fill-in text boxes for your convenience. Changes made to these forms can be saved with the Adobe Reader.

Applicants must submit either an Individual Property Nomination Form or a District Nomination Form, with accompanying documentation, to the address listed on the nomination form.

Staff reviews the application and submits it to the History Commission for a vote at an upcoming meeting. A file for each resource on the Inventory is kept at the Fairfax County Department of Planning and Zoning.
