service plan, and coordinates the connection to available programs
and services to assist residents in addressing their immediate
and long-term needs. CSP staff explores prevention and early intervention
strategies with residents and public and private human services
agencies, to address residents’ underlying needs in an effort
to assist individuals and families achieve economic independence
and social stability. Coordinated Services Planning provides information,
referral, and linkage to public and private human services available
to Fairfax County residents.
, residents may
call the Coordinated Services Planning call center to speak with
a Social Worker. Services are available in multiple languages
through the use of interpreter services. Coordinated Services
Planning multilingual staff are available to assist callers in
the following languages: Arabic, Bangla, Creole, Farsi, French, Hindi,
Korean, Punjabi, Somali, Spanish, Urdu, and Vietnamese.
• A Social Worker will listen and provide you with the
information you are seeking. In some cases, this may be a simple
phone number. In other cases, more in-depth work with a Social
Worker may be warranted.
• A Social Worker will assess your needs and work with
you to develop a personalized service plan. The goal of the
service plan is to help solve the crisis by connecting you with
the services and resources available within your community.
Self-sufficiency is encouraged and promoted.
• A Social Worker will explore prevention and early intervention
services and strategies available through community-based organizations
and Fairfax County service providers. The Social Worker will
offer guidance and provide service alternatives utilizing a
variety of resources.
Emergency food, shelter, clothing, furniture, and transportation
to medical appointments.
Rent or mortgage assistance to prevent eviction or foreclosure,
security deposit assistance to secure permanent housing, utility
payments to prevent disconnection, prescription assistance,
referrals for eye glasses, and other special needs. Assistance
is determined on a case-by-case basis and is generally available
only once a year.

Complete enrollment or make referrals for enrollment
to the Northern Virginia Dental Clinic based on residents’
zip codes. Provide referrals for other health care services—medical,
dental, pregnancy, and family planning.
Initial screening to secure in-home services, nursing home placement,
or to access other related services provided by the Department
of Family Services Adult Services Division.
Homeless Shelter Eligibility Screening for families of Fairfax
County who are seeking shelter placement and who do not have
a Social Worker with the Department of Family Services
Assistance with completion of Section
8 and Public Housing applications for residents who are
unable to navigate the online registration process.
Coordinated Services Planning staff has extensive knowledge
of resources available to meet the needs of Fairfax County residents.
These include services provided by Fairfax County agencies,
State and Federal agencies, non-profit organizations, community
based organizations, and some private for-profit organizations.
