Notice of Potential Release of Personal Patient Information

The Fairfax County Health Department has contacted or attempted to contact all patients whose personal health information (PHI) was potentially lost in the U.S. mail. On November 4, 2013, the Fairfax County Health Department mailed the computer disk containing pharmacy files to the Virginia Department of Health (VDH). On November 19, 2013 the VDH notified the Health Department that it had not received the disk. Upon investigation of this incident, the Health Department confirmed that the disk had been mailed to the accurate VDH Post Office Box; however, VDH has no record of receiving it.

The disk containing a pharmacy file included patient name, birthdate, address, medication, date of prescription, quantity dispensed, medication National Drug Code number, and physician’s name. Social Security Numbers (SSNs) were not included in the pharmacy file.

The Health Department takes this potential breach very seriously and therefore continues to work with the U.S. Postal Service and VDH to locate the disk. However, the Health Department has proactively contacted or attempted to contact affected patients rather than delay notification based on the hope that the disk will be recovered.
