Community Health Specialist

Description: In coordination with the Health Outreach Team, develops implements and evaluates community outreach and education focused on health promotion, prevention and health communication and emergency preparedness programming to the general public and in the Vietnamese, Chinese, and Korean communities.  This position provides specialized outreach and programming to non-profit organizations, hard to reach populations, parent and caregiver groups, small businesses and faith communities.  Creating and sustaining partnerships is a substantial function.  Aids in the evaluation of agency strategies and partnerships used to leverage community assets and improve population health metrics. Work will center on providing population-based interventions aimed at addressing the root causes and social determinants of health known to negatively impact health outcomes.  The position administers health surveys, supports data collection and informs written reports.  The Community Health Specialist ensures health education and communication services are provided in compliance with agency standards of care and local, state and federal regulations. The Community Health Specialist performs outreach to Fairfax County, Falls Church and Fairfax City and advances the Mission, goals, and objectives of the Fairfax County Public Health Department.

Minimum Qualifications: Any combination of education and experience equivalent to graduation from an accredited four-year college or university with a bachelor’s degree in health education, human services, or a directly related field;  plus one year of experience in communicable and/or chronic disease education, investigation, or other types of health-related work involving considerable public contact.

Special Language Requirements: Bilingual experience performing outreach work in the Vietnamese, Chinese or Korean communities, focused on health promotion and prevention. Bilingual capabilities, both written and oral, in the languages spoken by the targeted ethnic communities, must be completely fluent in English and Vietnamese, or English and Chinese or English and Korean. Must be able to speak, read and write English and Vietnamese or from English and Chinese or English and Korean    

Special Requirements: The appointee will be required to complete a criminal background check and a driving record check to the satisfaction of the employer.  The appointee must possess a valid driver’s license and will be required to travel to various locations within Fairfax County using own transportation.

Preferred Qualifications: Master’s degree in education or related fields is highly desired. Familiarity with Fairfax County is a plus.

Intellectual Disability Specialist I (Part-Time)

Two part-time IDS-I positions. Each position will work approximately 16.0 hours per week providing support coordination services to individuals with an intellectual disability who have Medicaid.   The duties will include providing ongoing support coordination services through monthly collateral contacts and face-to-face visits either every 90 days or every 30 days if the individual qualifies for the Enhanced Case Management.  Completes assessments, leads interdisciplinary team in developing Person-Centered Plan with the individual, links to needed services/supports, monitors delivery and quality of services according to the plan, is aware of person's interests and goals, acts as a resource to the individual and his/her family, meets with individual in the community, where services are provided, manages emergencies, and collaborates with vendors to ensure that health and safety needs are met. Work  must comply with all Medicaid requirements, time-lines and documentation.  Work requires the ability to enter information into an electronic healthcare record system. Case load size is about 8-10 persons.

Minimum Qualifications: Graduation from an accredited college or university with a bachelor's degree in a human services field, or equivalent experience.
Preferred Qualifications:  At least two years of experience working with persons who have intellectual disabilities, understanding of Medicaid and Medicaid Waivers, and excellent written and verbal communication skills. Qualified Mental Retardation Case Manager (QMRCM).

Licensed Practical Nurse (LPN) (part-time)

Description:  Exciting opportunity to work with multidisciplinary team at the Woodburn Center, in a community-based outpatient setting. Staff is being enhanced and expanded to provide integrated services, addressing substance use, mental health, and intellectual disabilities as well as co-occurring primary care needs.  Nurse will  perform behavioral health and primary care screenings, conduct wellness, psycho-education and medication groups. Management of medication clinic includes administration of medication; documentation and record keeping; observation, communication, coordinating prior authorizations and scheduling of appointments; taking vital signs and entering into Electronic Health.

Minimum Qualifications:  Graduation from an accredited Practical Nursing program; plus two years of experience as a Licensed Practical Nurse. 

Special Requirements:  The appointee to this position will be required to complete a criminal background check, an Office of the Inspector General (OIG) check and a check of the Child Protective Services Registry to the satisfaction of the employer. TB test is required. Must be licensed as a LPN (licensed practical nurse) in the state of Virginia.

Clinical Psychologist

Licensed Clinical Psychologist wanted  to conduct independent evaluations of adults and minors who have been involuntarily hospitalized under a court-issued Temporary Detention Order (TDO).  Duties include conducting comprehensive clinical evaluations, completing written reports, serving as an expert to the General District Court and the Office of the Fairfax County Attorney, and providing testimony at commitment hearings. Variable work schedule but must be able to work some evening hours and, once a month, some weekend hours. Training on relevant Virginia statutes and the role of the "Independent Evaluator" is provided.  This is a perfect opportunity for a clinician who wants to supplement their private practice and gain experience working in a forensic setting.  

Special Requirements: Possession of a current license to practice as a Clinical Psychologist in the Commonwealth of Virginia.
