In response
to adjustments to the Pay for Performance system reflected in
the FY 2004 Adopted Budget Plan, the Employee Performance Evaluation
Form has been revised for FY 2004. The only change on the form
is to page 8 which shows the revised pay and bonus amounts related
to a particular numerical rating.
The form
is available here in MS Word format. Click on the links below
to download the appropriate form. Please note that in order
to accommodate performance reviews effective prior
to payroll #14 (June 28, 2003) the old evaluation form is
still available for download. In addition, for those who have
already printed evaluation forms for performance reviews effective
with payroll #14, 2003 and beyond, and simply need to replace
page 8, that page is available as a separate download.
The revised form:
- Requires you to type information in section
I - Identification and Review Date Boxes
- Requires information in a footer which
begins on page 2 of the form.
To get a 2nd page footer:
- Go to VIEW
- Choose HEADER AND FOOTER and the "Header
and Footer Tool Bar" appears.
- On the "Header and Footer Tool
Bar" go to the 1ST ICON FROM RIGHT (SHOW NEXT) before
Close button. This brings you to the second page where the
Footer begins.
- Go to 3RD ICON FROM RIGHT (SWITCH BETWEEN
HEADER AND FOOTER). This brings you to the Footer section.
- TYPE in all information
- Hit CLOSE button. This will return you
to first page and out of the Header and Footer.Information
will appear on all consecutive pages in the footer excluding
page 1
- Rating categories are listed vertically
rather than horizontally.
- The final two (2) pages have been reordered
and section descriptions clarified.

PLEASE NOTE
The Employee Performance Evaluation
form has been formatted for an HP LaserJet 5si printer.
If another printer is set as your default printer, you
may need to adjust the page breaks for this document to
print correctly.
