Please report lost or stolen cards immediately to avoid being held responsible for materials checked out to your card by a third party. There is a $2.00 fee to replace a library card.
Replacement cards must be obtained in person at a library branch. Be prepared to show valid identification to replace a lost or stolen card.
Please complete the following information regarding your library account. If you are completing this form for your minor child, please be sure to enter the child’s information. Note any concerns or questions in the Comments section before submitting the form.
The library will use the personal information you provide solely to assist us in managing your account.
- Library cards are free of charge.
- Cards may be used at any of the 23 Fairfax County Public Library locations.
- Proof of home address is required. Please see eligibility requirements.
- Replacement cards are available for a $2.00 fee.
