Required Forms

To apply for a permit, you must submit three completed forms, and pay the required fees, to the Office for Children:

1. The application;
2. The Child Protective Services form;
3. The Sworn Statement/Affirmation form.

All three forms and payment must be submitted at the same time.

Here are detailed instructions about the three required forms, and the fees. Here is a checklist you can use to make sure you correctly submit the forms and fees.
