HOUSING - FAMILIES WITH ADULT WITH COGNITIVE/MENTAL DISABILITY
Four (3-4 bedroom) townhouses are available as permanent housing for families in which an adult has mental/cognitive disabilities. An assessment must confirm the mental/cognitive disability. Participants must be willing to develop and follow a service plan with the help of an agency case manager with whom they will meet on a weekly basis. Participants must be employed or participating in job training or an education program. Any recommendations for treatment must be followed. While this has some of the components of a transitional housing program, there is no time limit on length of stay; participants may remain in the housing as long as they follow their service plan.
