CASE MANAGEMENT - MULTICULTURAL
Develops case plans for the evaluation, treatment, and/or care of low-income, English-limited immigrants and refugees for whom existing services are inaccessible due to language and cultural barriers. Assesses the individual's needs then coordinates the delivery of needed services and ensures that services are obtained in accordance with the case plan. Follow up and monitoring of client's progress is integral. Housing, food, transportation, employment, and medical matters are just a few of the areas where a person may receive assistance.
