CASE MANAGEMENT - ADULTS WITH MENTAL ILLNESS
Provides outreach, supportive counseling, and case management services to adults who are homeless and mentally ill, some with concurring substance abuse. The primary focus is on assessing individuals' physical and mental needs and assisting in securing entitlements, obtaining permanent housing, employment counseling, and providing basic comfort needs. Counselors also coordinate linking individuals to other ongoing support services available in the community.
