Immunization and Vaccinations

The diseases that vaccines prevent can be dangerous, or even deadly. Vaccines reduce the risk of infection by working with the body's natural defenses to help it safely develop immunity to disease and are proven to be safe and effective. Protect yourself and your community. Make sure your family's immunizations are up-to-date.

Vaccinations are available through your health care provider and the Fairfax County Health Department clinics. Bring all immunization records to your appointment, including your yellow travel card if you have one.

Childhood & Required School Immunizations

Protect your child at every age. Find out what immunizations are recommended from birth through age 18.

Adult Immunizations

Vaccines aren't just for kids. Learn what immunizations you need as an adult.

International Travel Immunizations

The Health Department provides pre-travel consultations and many immunizations that are recommended prior to international travel for adults and children.

Immunization Forms

To shorten your wait, call ahead to make an appointment. Then fill out and print the following forms and bring them and your immunization records to your appointment at the Health Department.

If you are unable to print these forms, they will be provided to you at the time of your appointment.

Finding Your Immunization Records

If you received immunizations at a Fairfax County Health Department clinic, you can contact any clinic to ask about getting your immunization records. If you received your immunizations elsewhere, see CDC's tips for locating your immunizations records.
