The goal of the Service Coordination program is to link residents of housing who may be experiencing a crisis or hardships that jeopardizes their housing stability to supportive services in the community. When residents are referred to the PROGRESS Center, they are assigned a Service Coordinator who will meet with them to gather a brief history of services and conduct an assessment. Based on the resident’s needs, the Service Coordinator will then work to connect the resident to services, programs, and organizations in the community that can address those needs on an ongoing basis.
The PROGRESS Center does not accept outside or self-referrals. Referrals are accepted from the following providers:
HCD program staff (your Housing Specialist, Occupancy Specialist, Property Manager);
Coordinated Services Planning (CSP);
Fairfax County Health and Human Services agencies (Department of Family Services or Fairfax County Health Department) on a case-by-case basis;
Non-profit case management and social services providers (Britepaths, Northern Virginia Family Services, etc.) on a case-by-case basis.
