Support Coordination Services are activities designed to assist the individual child or adult in accessing needed medical, psychiatric, social, educational, vocational, residential and other supports essential for living in the community and other supports essential for living in the community and in developing his/her desired lifestyle. Case load size is about 28 persons.

Provides support to persons with intellectual disabilities who have Medicaid /Medicaid Waiver services. Completes assessments; leads interdisciplinary team in developing Person-Centered Plan with individual; links to needed services/supports; monitors delivery and quality of services, according to the plan and changes plan, as needed. Serves as a resource to the individual and his/her family and provides information about county, generic and community resources. Meets with individual in the community, where services are provided. Understands Medicaid Waiver services and complies with all Medicaid requirements, timelines and documentations. Counsels individual to enhance his/her problem-solving skills and develops positive relationship with the individual. Collaborates with vendors to ensure that health and safety needs are met. Manages emergencies. Must have excellent written and verbal communication skills and ability to negotiate agreements in difficult situations.



Any combination of education, experience, and training equivalent to graduation from an accredited college or university with a master’s degree in psychology, social work or related field.



The appointee to this position will be required to complete a criminal background check, OIG check and a check of the Child Protective Services Registry to the satisfaction of the employer. Valid driver's license. Ability to use personal computer and MIS.



Experience in education, psychology, social work or related field, with a minimum of two years experience working with persons who have intellectual and other disabilities.



Panel interview.



Hours are from 8:00 a.m. to 4:30 p.m. with flex hours based on agency need. Rotating on-call responsibility. Limited telework may be available.
