Description:

This case manager/treatment provider primarily serves the most seriously mentally ill older adults with a wide range of psychiatric disorders, including co-occurring disorders with addictions; intellectual disabilities; physical challenges; dementia and/or other disorders. Functions autonomously as a licensed primary case manager performing a comprehensive array of professional treatment services for the most acute mental health population, requiring extensive case management and clinical expertise. Serves a case load of diagnostically, culturally, and socio-economically diverse clients. Also functions as a member of a multidisciplinary treatment team. Performs independent clinical assessments, including those for Medicaid. Duties are performed using a history of strong collaborative relationships and ability to work with consumers in the community utilizing their natural supports; strong/expert level case management skills; knowledge of CSB, Fairfax County and community resources; adherence to Recovery Model; strong co-occurring disorder assessment/treatment skills; significant experience providing individual and group treatment to include evidence-based, cognitive/behavioral orientations. Schedule includes at least one evening per week, depending on site-based needs.

 

Minimum
Qualifications:

Graduation from an accredited college with at least a master's degree in social work, psychology, counseling, or nursing; plus two years of clinical experience.

 

Special
Requirements:

Possession of a current license to practice in the Commonwealth of Virginia in one of the following: Licensed Clinical Social Worker, Licensed Professional Counselor, Clinical Psychologist, Licensed Marriage and Family Therapist, or Clinical Nurse Specialist. TB screening upon hire and annually thereafter.

The appointee to this position will be required to complete a criminal background check to the satisfaction of the employer.

 

Preferred
Qualifications:

Combination of training and/or experience providing services to SMI older adults. Knowledge of the aging population, particularly mental health and developmental issues common to that population. Requires crisis intervention skills; strong engagement and outreach skills; and a willingness to provide supervision to student interns and staff for licensure. Requires strong computer skills and the ability to navigate an Electronic Healthcare Record.
