JAIL DIVERSION PROGRAM-INTENSIVE CASE MANAGER
(Mental Health Therapist)

Description:

Position is based at the Woodburn Mental Health Center. Provides intensive case management and mental health support services to adult individuals with serious mental illness/co-occurring disorders who are involved with the criminal justice system. Work occurs primarily in the community and at the Fairfax County Adult Detention Center. Evening work may be required based on program and consumer need. Works collaboratively with other members of the Jail Diversion team as well as community-based partners, day programs, supportive employment and crisis intervention services. Functions as primary single point of contact for services. Refers, links, advocates and provides support to a vulnerable, high-needs population. Provides comprehensive assessment of clinical and case management needs and partners with individuals to develop and implement a recovery-based service plan. Links individuals to benefits, housing, vocational opportunities, medical services and transportation. Provides crisis intervention and stabilization as needed. Documents individual contact and services according to program, licensure and Medicaid requirements.

 

Minimum
Qualifications:

Graduation from an accredited college with at least a bachelor's degree in social work, psychology, sociology, counseling, nursing (or, for certain positions providing services to children and adolescents, a bachelor's degree in special education) plus two years of clinical experience or graduation from an accredited college or university with a master's degree in social work, psychology, sociology or counseling plus one year of clinical experience. Clinical experience means providing direct clinical services to individuals with mental illness and includes supervised internships, practicums, and field experience. Additional education may not substitute for clinical experience.

Requirements:

All employment offers are contingent upon the completion of a criminal background check, an Office of the Inspector General (OIG) check and a check of the Child Protective Services Registry to the satisfaction of the employer.

 

Preferred
Qualifications:

Ability and willingness to provide services to the most acute and vulnerable mental health population. Must be able to transport clients safely; must have a good driving record.
