Description:

The position is with the Program of Assertive Community Treatment (PACT) located in the Mount Vernon area of Fairfax County.

As a member of a 14-member interdisciplinary psychiatric outreach team, provides rehabilitation, clinical case management, crisis intervention and stabilization, support services, and supportive counseling to persons with serious and persistent mental illness served by PACT. Recovery-oriented services are provided via individual, side-by-side, family, and group modalities in mostly non-traditional community/outreach venues (e.g., unsheltered settings, homeless shelters, jails, hospitals, client homes, job sites). Serves as the Transition Specialist on the team by assisting individuals to develop a plan to transition to a less intensive level of care and provides support, reassurance, and guidance as they transition out of PACT. The Team includes clinical case managers, psychiatric nurses, psychiatrists, and vocational, substance abuse, peer, transition, and residential specialists. In addition to weekday daytime shifts, this position works weekend, evening, and holiday hours on a rotating basis and may include on-call coverage.

 

Minimum
Qualifications:

Graduation from an accredited college with at least a bachelor's degree in social work, psychology, sociology, counseling, nursing, plus two years of clinical experience; or graduation from an accredited college or university with a master's degree in social work, psychology, sociology or counseling plus one year of clinical experience. Clinical experience means providing direct clinical services to individuals with mental illness and includes supervised internships, practicums, and field experience. Additional education may not substitute for clinical experience.

 

Special
Requirements:

The appointee to this position will be required to complete a criminal background check, an Office of the Inspector General (OIG) check and a check of the Child Protective Services Registry to the satisfaction of the employer. Valid motor vehicle driver's license required.

 

Preferred
Qualifications:

Experience working with individuals with serious mental illnesses and co-occurring disorders in a community setting. Knowledge of and experience working with individuals who are, or have been homeless, incarcerated, or hospitalized is a plus. Knowledge of motivational interviewing. Computer literacy.
