Description:

Support coordination/case management.
This position is under consideration for being moved to the Merrifield Center when the Center opens. Provides leadership, supervision, guidance and support to seven Support Coordinators/Case Managers (SCs/CMs) who provide coordination services to 25 assigned individuals receiving ID Medicaid Waiver, State Plan Option (SPO) and/or active support coordination services. Provides training, clinical and technical support to SC/CMs, completes performance evaluations, and has knowledge of personnel regulations, operational procedures and Medicaid regulations. Ensures that SC/CMs are in compliance with Medicaid Waiver/SPO requirements. Completes utilization reviews; covers cases in the absence of a SC/CM; and manages or assists with individual emergencies. Is part of an on-call rotation for after-hour emergencies. Understands and uses MIS for electronic health records. Assists with prioritization of workloads; assigns tasks; monitors compliance with and initiation of state, county and Community Services Board initiatives; and ensures that quality support coordination/case management is provided to those served. Duties are performed under the direction of the Assistant Director, Support Coordination.

 

Minimum
Qualifications:

Any combination of education, experience, and training equivalent to a graduation from an accredited college or university with a master's degree in psychology, social work, or a related field; plus three years of related professional experience; or graduation from an accredited college or university with a doctoral degree in psychology, social work, or a related field.

 

Special
Requirements:

The appointee to this position will be required to complete a criminal background check, an Office of the Inspector General (OIG) check and a check of the Child Protective Services Registry to the satisfaction of the employer. Valid driver's license.

 

Preferred
Qualifications:

Master's degree in psychology, education, social work or related field. Knowledge of intellectual disabilities. Five years of experience working with persons who have intellectual disabilities. Excellent written and oral communication skills. Understanding of Medicaid and Medicaid waivers. Experience supervising a team of case management professionals.
