Chronic Disease Self-Management Program (CDSMP) and
Diabetes Self-Management Program (DSMP)

This free six-week workshop will help you:
  • Feel better
  • Increase energy
  • Get relief from pain and fatigue
  • Do the things you want to do each day

Who should attend?
  • Adults with ongoing health conditions
  • Friends, relatives, or caregivers of a person with chronic illness
  • Chronic illness can include arthritis, heart problems, asthma
         and other respiratory conditions, diabetes, and high blood pressure

What to expect at the workshop?
  • 8-12 participants with a wide range of chronic health issues
  • Workshops led by two trained leaders who have chronic illness
  • The opportunity to give and receive support
  • A copy of "Living a Healthy Life with Chronic Conditions"

Fairfax County's Diabetes Self-Management Program is a free 6-week group interactive workshop series for individuals concerned about living with prediabetes or diabetes. Learn and share positive strategies to better manage overall health, increase self-confidence and improve the skills you need to take control of your health!
