The Fairfax County Community Health
Care Network is a partnership of health professionals, physicians,
hospitals and local government. It was formed to provide
primary health services for low income, uninsured County
residents who cannot afford primary medical care services
for themselves and their families.
Applications are taken in person at the health
centers. The list of enrollment requirements for the Community
Health Care Network in English and Spanish are available.
