Change in Information, Address and Adults Living or Working in the Home

You must report any change of information to the Office for children within 21 days. Promptly send in updated information and documents to: Fairfax County Office for Children Community Education and Provider Services Suite 820 12011 Government Center Parkway Fairfax, VA 22035-1104 If You Move Your family child care permit is issued for your specific address, not for you as a person. Therefore, if you move to a new address, a new permit must be issued. You will need to complete: A new Home Child Care Facility Application (submitted with the $14 fee); A fire and safety visit from the Fairfax County Fire and Rescue department; A home health and safety visit from the child care specialist. Your new home must be inspected within 15 days of your move. Change in Information If you change your phone number or email address, please call and update this information with your Administrative Coordinator. You can also submit your changes by mail on a Child Care Central Website Application form. Change in Adults Living or Working in the Home If anyone age 18 or older moves into your home after you have received your child care permit, you need to submit these required documents: Sections C, D and E of the Home Child Care Facility Application form - English . . . Spanish Sworn Statement or Affirmation for Child Day Programs form - English . . . Spanish Child Protective Services Central Registry Release of Information form - English . . .
