Your child between the ages of 0 to 18 may be eligible if your family:
- Meets income guidelines
- Does not have affordable health insurance through a parent’s employer.
- Does not qualify for Medicaid (FAMIS Plus) and FAMIS.
Note: Citizenship is not required.

What to Expect After You Return Your Application

A case worker will call you within two days of receipt of the application to review it with you.

If you do not return all necessary information, your application will be denied 45 days following the date your application is received. Please call your worker if you need assistance with collecting the required information.
