MotivateMe eligibility may vary by individual circumstances, such as Medicare Advantage plan enrollment. Please contact LiveWell@fairfaxcounty.gov to discuss your eligibility or with any questions.
Cigna Members: Activities must be completed AND posted in the Cigna MotivateMe portal by December 31st at 11:59 PM.
Annual physicals and cancer screenings can take up to 8 weeks to post as complete in the MotivateMe portal on MyCigna.com. We recommend having claims-based services completed by October. It is the subscriber's responsibility to allow for adequate processing time prior to the deadline.
Self-reported activities can take up to 72 hours to post as complete in the MotivateMe portal on MyCigna.com. It is recommended to submit these prior to December.
Cigna members do not need to wait the complete 365 days to have their annual physical. Subscribers may receive one annual physical per calendar year.
Have a discussion with your provider and their office staff regarding the coding of your visit (must be coded as a preventative exam), and the billing timeline (some offices may bill less frequently).

Kaiser Members: Kaiser passports must be emailed to LiveWell@fairfaxcounty.gov by December 31st at 11:59 PM.
A doctor's signature is required for the annual physical activity. Please be sure to print out your passport and take it with you to your appointment. You can also provide a doctor's note or after visit summary containing your name, the date of service, and the service provided.
Self-reported activities must be dated and initialed by the plan subscriber.
Passports are accepted on a rolling basis every year. It is never too early to submit your passport, and it can always be updated (prior to December 31st).
Passports can be turned in via fax (703-802-8795), email- scanned/photo (LiveWell@fairfaxcounty.gov), or in-person (Govt. Center Suite 270).
Medicare
Claims-based activities such as an annual physical and cancer screening paid by Medicare may not be recognized by the Cigna system. Please send LiveWell@fairfaxcounty.gov one of the following by December 31st. at 11:59 PM.
Explanation of Benefit (EOB) that states your full name, provided service, and the date of the service.
Documentation from the doctor on a script pad or letterhead stating your name, provided service, date of service, name of doctor, and signature of doctor.
Please note: we do not need medical information such as test results, diagnostics, or other personal information.
