• MotivateMe eligibility may vary by individual circumstances, such as Medicare Advantage plan enrollment. Please contact LiveWell@fairfaxcounty.gov to discuss your eligibility or with any questions.
• *As of 2026, Kaiser members are no longer eligible to particiapte in the LiveWell MotivateMe program, as Kaiser introduced their own incentive reward program. Kaiser members should visit Kaiser Permanente or contact Kaiser customer service to learn more and get started earning rewards.

  • Keep track of your activities and the dates you complete them (you must complete ALL activities)
  • Complete the NEW Submission Form by November 30th, 2026
    • Be sure to select the checkbox to have your submission confirmation emailed to you. This with the email date and time stamp will act as your receipt.
