Integrating public health data to improve monitoring, analysis, reporting and evaluation of community health.
An essential function of a public health system is to provide data and information about the health of its community members. Data on health-related behaviors, diseases and injuries and causes of death can help decision makers identify and address health problems more effectively. These types of data are monitored by many entities at the local, state or national levels through existing public health surveillance activities. In addition, information about the social factors that influence health can help frame a better understanding of the issues that impact health outcomes.
Community Health Improvement Plan (CHIP)
Data Goal: Develop recommendations for a comprehensive public health data collection, monitoring, analysis,and reporting system to support evaluation of health outcomes.
In the Fairfax community, data sources are abundant, but often fragmented across multiple systems and sectors with distinct owners. Addressing the gaps in data collection, monitoring, analysis, reporting and evaluation can help improve public health efforts aimed at reducing health disparities and improving the health of the community as a whole.

What's Happening in the Community?
The Live Healthy Fairfax Community Health Dashboard is a web-based data resource center for the Fairfax community. It provides local socioeconomic and health data, links to data resources, best practices, and other information to promote community health and well-being. The intent is to give the community the tools needed to understand the public health indicators that affect the quality of life for all who live, work and play in Fairfax. The Dashboard provides a wealth of non-biased information in one, accessible, user-friendly location. It provides information on a broad range of topics affecting the health of our community and highlights the assets in our community. The data is updated whenever source data is updated, providing the most up-to-date information of its kind.
The goals of the Community Health Dashboard are:
- To make local community data more accessible to the community.
- To provide state-of-the-art, web-based assessment and improvement tools to encourage data-informed decisions that influence health and well-being in our community.
