Refund of Contributions Forms

If you have left employment before retirement, you have options regarding the contributions that you have made by payroll deductions to the retirement system. We recommend that you review the Options for Terminating/Separating Employment. If you are vested (have 5 or more years of service), we recommend that you review the Deferred Vested Retirement section of the Retirement Handbook.
