Active Employee

FOR EMPLOYEES HIRED ON OR AFTER JULY 1, 2019

Beneficiary designation forms may be mailed or faxed, but a wet signature or electronic signature is required. It is not acceptable to type your name in the signature box.

Preparing for Retirement

When you are nearing retirement, you can request that our staff to prepare an estimate of your retirement benefits. To do so, fill out the appropriate form below depending on your employer.

When you are ready to retire, please contact your retirement analyst or complete a form to begin the process.

Schools Employees

If you work for Fairfax County Public Schools (FCPS) and are a member of the County's Employees' Retirement System, please contact your retirement analyst based on the first letter of your last name:

If you are vested (have 5 or more years of service) but are not eligible for early or normal retirement, you may be eligible for a deferred retirement benefit. To apply for this benefit, complete one of the following forms:

If you are an active employee and have reached early or normal retirement eligibility, you can use the following forms to designate a beneficiary to receive benefits in the event of your death.

Entering the Deferred Retirement Option Program (DROP)

If you are eligible for normal retirement, you have the option of entering the Deferred Retirement Option Program (DROP). To do so, use one of the following forms to notify us that you want to enter DROP.

Exiting the Deferred Retirement Option Program (DROP)

If you are in the Deferred Option Retirement Program (DROP) and are ready to exit and retire, complete one of these forms.

If you left employment before retirement, you have options regarding the contributions that you have made by payroll deductions to the retirement system. We recommend that you review the Options for Terminating/Separating Employment. If you are vested (have 5 or more years of service), we recommend that you review the Deferred Vested Retirement section of the Retirement Handbook.

When you are ready to request a refund or rollover of your contributions, use this form: Acknowledgement of Employment Termination.

Please know that there will be tax consequences if you take a refund of your contributions, so we recommend that you read this Special Tax Notice.

If you are the spouse of a deceased Fairfax County employee in the Employees' or Uniformed Retirement Systems, use this form: Spouse Refund/Benefit Request.

Retiree

  • If you are retired and either need to set up or change your direct deposit information, please use this form: Direct Deposit

Please note that the federal Form W-4P form was new as of 2023.  If you need to make changes to your federal tax withholding, you must use this new form. If you wish to learn more, use this link to the IRS.
