Guide to paying for CSB services

Fees

The Fairfax-Falls Church Community Services Board (CSB) charges fees to help offset the cost of services. We collect financial and insurance information to determine appropriate fees, which are reviewed at least annually or whenever your circumstances change.

The CSB accepts cash, check, money order, and credit cards for payment at the time services are provided. The CSB also offers an online payment portal. The CSB will file health insurance claims on your behalf with most insurance plans. Your liability will be determined based on the percentage not covered by the Ability to Pay Scale.

Health Insurance and Billing

The CSB accepts and contracts with a variety of insurance plans. For most plans, we will file claims on your behalf. Your financial responsibility (copayments, coinsurance, deductibles, and any remaining balance) will depend on your specific plan, household income, household size, and the Federal Poverty Guidelines.

We recommend contacting your insurance provider to confirm:

  • If CSB is an in-network provider
  • Your coverage and benefit details
  • Your copayment, coinsurance, and deductible obligations

We will submit claims for both in-network and out-of-network plans, though out-of-network services may result in higher out-of-pocket costs.

Fee for missed appointments

If you cannot attend a scheduled appointment, please notify your site’s front desk, case manager, or therapist at least one day in advance. If you fail to do so and do not show up, you may be charged a $25 missed appointment fee.

What to bring to your first visit

To help us complete your registration and financial intake accurately and efficiently, please bring the following items. Bringing these documents allow us to process your registration more quickly. For examples of valid documents, see our list of acceptable documentation

Identification and Residency

  • Proof of identity (e.g., driver’s license, passport, or state ID)
  • Proof of residency in Fairfax County, Fairfax City, or the City of Falls Church
  • Health insurance cards, including Medicaid and Medicare
  • Contact information for your current primary care physician and/or therapist
  • Proof of legal Virginia guardianship and/or guarantor assignment, if applicable

Income Documentation
If you have income, please bring:

  • Most recent income tax return (preferred)
  • Recent pay stubs for the past month for yourself and your spouse, if married
  • Most recent Social Security Annual Earning Statement, 1099 form, or award letter

Note: We may request your permission to contact your employer to verify any missing pay information.

No Income
If you have no income, you will need to complete and sign an attestation confirming that your information is accurate to the best of your knowledge.

Financial Assistance & Ability to Pay

The CSB offers financial assistance for eligible residents of Fairfax County, Fairfax City, and the City of Falls Church. Assistance is based on a sliding fee scale determined by the CSB’s Ability to Pay Scale, which takes into account your household income and size.

Basic Fee Subsidy/Liability

A basic fee subsidy may be applied to services not covered by your insurance plan. It is determined using your reported household income and size. Please note:

  • This subsidy does not apply to copayments, coinsurance, deductibles, or any other costs deemed your responsibility by your insurance provider.

Supplemental Fee Subsidy/Hardship

If full payment of fees presents a documented financial hardship, you may be eligible for an additional subsidy. This supplemental assistance requires approval from your CSB provider or the Revenue Management Team (RMT).

Payment options

Payment is expected at the time of service or once insurance payments have been processed and within 30 days of receiving the service invoice. The CSB accepts the following forms of payment:

  • Cash
  • Check or money order (there is a $50 service fee for returned checks).
  • Credit cards
  • Online payments via our Make a Payment Online

In the event that your account becomes past due, the balance is considered as delinquent. Delinquent accounts may be placed for collection. Should your account become past due, you will be subject to the full outstanding balance and any applicable collection agency fees. Collection agency fees can range from 20%-30% more than your original balance.

If you are experiencing financial challenges, please speak with CSB billing staff about a payment plan or supplemental fee subsidy to avoid delinquency and subsequent collection process.

Annual review of fees

Fees are reviewed and established annually by the CSB Board and submitted to the Fairfax County Board of Supervisors. Your ability to pay is reassessed annually on your account anniversary date, or sooner if needed, to ensure the most accurate and current information is used to determine your fees. You will be required to submit updated financial documentation at that time. 

If your information changes before your annual review, please notify a member of our staff so your records and fees can be updated as appropriate.
