What to expect when you come for a walk-in assessment
Merrifield Center walk-in assessments
- Walk-in hours are Monday through Friday, from 9 a.m. to 5 p.m. Youth walk-in evaluations are offered during these times and also until 7 p.m. on Tuesdays.
- The Merrifield Center is located at 8221 Willow Oaks Corporate Drive, Fairfax, Virginia 22031, just off Arlington Boulevard at the corner of Willow Oaks and Williams Drive. [Directions]
- The building can also be reached by MetroBus (Routes 1Z, 1A and 1C), and Connector bus (Routes 401 and 402).
What happens during an assessment
- If you are an adult seeking an assessment, you will be asked to check in at the front desk on the building’s lower level. Check-in for youth assessments is on the third floor.
- You will be asked to fill out some paperwork and provide some brief medical background information to help us better understand the symptoms you are experiencing.
- You may be asked to wait while your information is being reviewed.
- You will then be invited to meet with a clinician who will ask more questions about your symptoms and history. The clinician will type the information into an electronic health record. This is your personal information and will not be shared without your permission.
- The clinician will also talk with you about whether you appear to be eligible for CSB services, or whether there might be another resource in the community that would be more helpful and appropriate for you.
- If you do not appear to meet CSB admission criteria, you will be given a list of other service providers and resources in the community. If you are unable to get help from these listed providers, please get back in touch with us and we will try to help you find other resources.
- If you do meet CSB admission criteria, you will then speak with a member of CSB’s business staff to go over fees and insurance information and conduct a business registration.
- You will then be invited to meet with a clinician for a more in-depth mental health and/or substance use assessment, which may take as much as two hours.
- After the assessment, you will receive a treatment recommendation. This could include various services, depending on the level of care that is recommended.
- If you decide to pursue the CSB services recommended for you, the clinician will explain the next steps toward starting treatment.
What to bring to your first visit
The following information is important to bring to your first visit, so that we can process your registration efficiently.
- Proof of identity (such as a driver’s license or other state-issued ID card, school picture ID, passport, picture ID issued by an embassy or consulate, or a birth certificate)
- Proof of residency in Fairfax County, Fairfax City, or the City of Falls Church (such as a lease agreement, letter from current landlord, mortgage agreement, DMV change of address card, driver’s license or other state-issued ID card with current address, utility bill with your name and current address, or a letter from homeless shelter if you are currently homeless)
- Health insurance cards, including Medicaid and Medicare cards
- Name, address and telephone number of your current primary care physician and/or therapist
- Proof of legal guardianship and/or proof of guarantor assignment, if needed
Also, if you have income, please bring:
- Most recent Social Security Annual Earning Statement (or 1099 form or award letter)
- Most recent income tax return (preferred)
- Recent pay statements (pay stubs) for the past month for self and spouse if married.
We may ask your permission to call your employer to obtain missing pay information.
If you have no income, you will be asked to certify that the information is true to the best of your knowledge.
