Obtaining a family child care permit is the first step toward becoming a family child care provider in Fairfax County.
To apply for a permit, follow the seven steps below, and submit all four forms and additional documentation to the Fairfax County Early Childhood Programs and Services. If you have any questions, call 703-324-8100, TTY 711.
- STEP 1 – Complete the Home Child Care Facility Permit Application. Include all requested information in each section.
- STEP 2 – Complete the Child Protective Services Central Registry Release of Information for each person, 14 years or older, who lives in the home where child care will be provided, including the applicant and all substitute care providers. This background search is repeated every five years.
- STEP 3 – Complete the Sworn Statement or Affirmation for each person, 18 years or older, who lives in the home where child care will be provided, including the applicant and all substitute care providers. This form is resubmitted every year at your renewal time.
- STEP 4 – Complete the Waiver Agreement and Statement for the Fingerprint National Background Check with a $27 fee for each person, 18 years or older, who lives in the home where child care will be provided, including the applicant and all substitute care providers.
- STEP 5 – Submit Completed Forms along with the required fees to CEPS for processing.
- STEP 6 – Additional Documents and Requirements After completing the four forms, you must comply with several additional permit requirements.
